Summary
Introduction: acute paracentral median maculopathy (PAMM) is an entity described in spectral domain optical coherence tomography (SD-OCT). It is characterized by the presence of a parafoveal hyperrefringent band in the middle retinal layers and is manifested by the appearance of paracentral scotomas. This article presents a clinical case with the diagnosis of PAMM and a brief review of the literature. Methods: Written informed consent was requested from the patient under the approval of the Ethical-Scientific Committee of the Faculty of Medicine of the Pontificia Universidad Católica de Chile. The review of the literature was carried out using a search strategy that included the terms specified in the keywords. Results: the patient consulted for a superior paracentral scotoma in the right eye (RE). Autofluorescence revealed a lower RE parafoveal hypoautofluorescent lesion. Retinal angiography showed slight leakage in the same area, with no signs of arterial occlusion. In the SD-OCT of the RE, an inferior parafoveal hyperrefringent lesion was observed in the middle retinal layers. The systemic study ruled out infectious-inflammatory and vascular causes. Based on the above, the diagnosis of PAMM was made. Conclusions: the clinical presentation and the results obtained were consistent with what was reported in the literature regarding PAMM. Management is currently based on searching for and controlling related conditions, since no specific treatment has been described. In this case, secondary causes were ruled out and clinical observation was decided.
